During the month of March, Hospices across the UK are joining together to support Open Up Hospice Care, an annual campaign run by Hospice UK.
The theme of this year’s campaign focuses on care in the community and in the patient's own home and aims to raise awareness of the support Northern Ireland Hospice provides outside of the hospice building.
We're aiming to raise awareness of two of our five models of care:
- Community Services
- Hospice Hub
Our Community Nursing team, consisting of Hospice Nurse Specialists, Hospice Community Nurses and Speciality Nurses in Palliative Care, care for people with life-limiting and life-threatening illnesses across Northern Ireland, providing high quality, person-centred and holistic palliative care.
To empower patients to live will with their condition and to remain as independent as possible for as long as possible at home, we’ve recently developed a new model of care called the Hospice Hub which aims to improve patients’ well-being at a time in their life when they are adjusting to the impact of their illness.
In 2017/18, we witnessed a 16% increase in referrals to this vital community service. Sadly, however, there are many more people across Northern Ireland who need hospice care and access to our vital community services, however, can’t get it.
With demand for hospice care set to increase, we need your support today to enable us to expand the reach of hospice care, offering more people support they need, wherever they need it.
